ABSTRACT

Introduction. Ces vingt dernières années, on assiste à une augmentation spectaculaire du nombre de cas de diabète de type 1 avec une mortalité plus élevée en Afrique Noire en lien avec les difficultés d'accès aux soins, à la rupture de suivi engendrant un nombre élevé de perdus de vue. L'objectif du travail est de déterminer le profil épidémiologique des enfants diabétiques perdus de vue dans notre pratique. Patients et méthodes. C'est une étude transversale à visée descriptive et analytique qui a concerné les enfants diabétiques perdus de vue de 2002 à 2019 dans le service de diabétologie du CHU de Yopougon. Résultats. Durant l'étude 195 enfants ont été suivi dans le service, 90 ont été considérés comme perdus de vue soit une fréquence de 46,15 %. L'âge moyen des perdus de vue était de 14,6 ± 5,3 avec des extrêmes allant de 2 à 20 ans. Ils sont majoritairement de sexe F dans 50,6 %. Dans 37 % on retrouve un niveau socio-économique faible calculé selon l'indice IPSE avec une association statistiquement significative entre les classes socio-économiques moyenneinferieure et moyenne (p respectivement < 0,01 et 0,001). La plupart des perdus (84,5 %) n'ont pas d'assurance maladie. Le lieu de résidence était urbain (Abidjan) dans 73 %. Ils ont issus d'une famille biparentale dans 79 ,8 % des cas. Une association statistiquement significative a été retrouvée entre la non-scolarisation et la perte de vue (P < 0,05).Conclusion. Le manque de suivi chez les enfants diabétiques a des conséquences drastiques sur le pronostic de la maladie au long cours, marqué par une mortalité de 11 % qui touche essentiellement les populations à revenu faible


Introduction. Over the past twenty years, there has been a spectacular increase in the number of cases of type 1 diabetes with a higher mortality rate in Black Africa due to difficulties in accessing care but also a lack of follow-up resulting in a high number of people being lost to follow-up. Our work aims to determine the epidemiological profile of diabetic children lost to follow-up. Patients and methods. This was a cross-sectional study with descriptive and analytical aims which concerned diabetic children lost to follow-up from 2002 to 2019 in the diabetes department of the University Hospital of Yopougon. Results. During the study period, 195 children were followed up in the department, 90 were considered as lost to follow-up, i.e. a frequency of 46.15%. The average age of the lost to follow-up was 14.6 ± 5.3 years with extremes ranging from 2 to 20 years. The majority of them were male (50.6%). Thirty-seven percent had a low socioeconomic level calculated according to the IPSE index, with a statistically significant association between the lowermiddle and middle socioeconomic classes (p < 0.01 and 0.001 respectively). Most of the lost (84.5%) did not have health insurance. The place of residence was urban (Abidjan) in 73%. They came from a two-parent family in 79.8% of cases. A statistically significant association was found between non-education and loss of sight (P < 0.05). Conclusion. The lack of follow-up in diabetic children has drastic consequences on the prognosis of the disease in the long term marked by a mortality of 11% which affects mainly low-income populations.

ABSTRACT

Introduction: Keeping HIV-infected adults away from the health care system during the COVID-19 travel restrictions, presents a challenge to HIV treatment adherence. Methods: This study focused on the initial two phases where Phase 1 designed a Makerere College of Health Sciences (MakCHS) Unstructured Supplementary Service Data (USSD)-based application; and Phase 2 piloted patient enrolment onto the application and determined the feasibility of remote follow-up of patients receiving long-term antiretroviral therapy (ART). Results: A off/online user application, MakCHS Health app, was developed. Overall, 112 patients [(66(59%) female] receiving ART at Mulago ISS clinic, Kampala, were enrolled onto the MakCHS Health app. Up to 89 (80%) utilized the app to access medical help. Patients' medical queries included needs for drug refills, missed taking HIV medication, medical illnesses, access to COVID-19 vaccination and other personal needs that required clinicians' attention. Conclusion: Piloting a MakCHS Health application for patient follow-up was feasible and well-received by HIV treatment providers and patients receiving ART. We recommend scale up of the application to enroll all patients receiving long-term treatment for HIV/AIDS, and subsequently expand to. other HIV treatment programs in similar settings

ABSTRACT Objective: This study aims to evaluate the level of patients' compliance with recall / follow-up visits attending the dental Centre of the University of Benin Teaching Hospital (UBTH).Methods: This study adopted a descriptive retrospective study of case notes retrieved from the medical records department of the dental Centre, University of Benin Teaching Hospital. The period under study covers 2019 to 2021. The data was collected by means of 1232 questionnaire and analyzed using descriptive statistics such as frequency and percentages. Results: The result showed that the participants in the age bracket of 18-40 years of age (277) complied better to recall visit in comparison with the study age distributions which are 41-65 years (218) and age above 65 years (188). As per the influence of gender on compliance of patients with post-extraction recall visits, it was observed that the male participants (288) responded better to the recall visits compared to the female participants (251). The Relationship between distance away from the hospital and compliance of patients with post-extraction recall visits was also evaluated, and it was observed that the patients closer to the hospital within 5km (442) complied better to recall revisit in comparison to the participants who were within 10km (224) from the hospital as well as the participants that were 10km & above (17). Finally, the influence of interval between procedure and recall visit was also evaluated, and the result revealed that the participants that were given 7 days of recall revisit (663) complied better compared with the participants who were given 14days duration for recall revisit (478). Conclusion: The data from this study highlighted the possible need for a paradigm shift in patient-doctor interphase especially as it concerns recall visits. Compliance to recall appointment by the patients depends largely on age, gender, distance from hospital and interval between procedures and is mainly responsible for the noncompliance of patients to recall visit

ABSTRACT

Introduction: La fulgurante progression de la pandémie à covid -19 a imposé au Sénégal l'adoption de stratégies de riposte parmi lesquelles la mise en place de centres de traitement des épidémies (CTE) au sein des hôpitaux . Nous nous proposons d'évaluer les activités d'un CTE Covid-19 implanté dans un service de médecine interne et les leçons tirées de ce vécu. Méthodologie : Le CTE Covid -19 a été installé dans le service de médecine interne de l'Hôpital Régional de Thiès (HRT), mais avec conservation de lits dédiés aux patients non atteints de Covid-19. Les étudiants en année de doctorat affectés dans le service de médecine interne étaient responsables de la gestion quotidienne du CTE sous la supervision des spécialistes en médecine interne Ce service était subdivisé en deux parties: le CTE qui prenait en charge les cas de Covid -19 et le reste du service qui devait continuer à accueillir les patients atteints d'autres affections ou qui y étaient régulièrement suivies. Résultats : Du 1er mai au 30 octobre 2020, 237 patients ont été admis dans le CTE. Ils étaient âgés de 7 à 88 ans avec une moyenne d'âge de 53,41 ans et un sexe ratio de 1,60. Les motifs d'admission étaient une désaturation en oxygène inférieure à 90%, la présence d'au moins une comorbidité (autres infections, diabète , hypertension artérielle , obésité, maladies auto-immunes, cancers…). L'âge avancé mais aussi les patients ne pouvant être à domicile faisaient également partie des critères d'admission . Trois (3) cas de co-infection Covid-19 et tuberculose pulmonaire ont été relevés et trois (3) patients avaient un portage chronique du virus de l'hépatite B. Dans le cadre des hospitalisations non Covid -19, les affections suivantes ont été retrouvées : 8 cas de diabète déséquilibrés et autant d'hépatopathie (6,10%); l'accès palustre dans 3, 05% (n=3) ; la tuberculose pulmonaire (3,81%, n=3) ; 3 cas (2,29 %) d'anémie de type biermerien et de lupus érythémateux systémique. De même, 1 cas (0,76%) d 'empyème cérébral ; une polyarthrite rhumatoïde (0,76 %), une (01) maladie rénale chronique , 1 cas de défaillance cardiaque ont également été enregistrées. Cinq (5) cas (3,81 %) non affectés par l'infection à Covid -19 , à leur admission l'ont été au cours de leur hospitalisation et donc transférés au niveau de la zone rouge du CTE. Conclusion: La mise en place du CTE au niveau du service de Médecine interne , a permis une adaptation efficiente dans la prise en charge des patients concernés mais aussi de ceux qui étaient suivis pour des pathologies chroniques comme les urgences médicales reçues durant la période. La continuité des soins a été assurée et les liens avec les autres secteurs de la pyramide sanitaire du Sénégal ont été raffermis.


Introduction : The fast progression of covid -19 throughout the world has forced Senegal to adopt response strategies including the establishment of Outbreak Center for Covid- 19 (OCC ) within hospitals . We propose to evaluate the activi ties of an OCC implemented in an internal medicine department and the lessons learned from this experience. Methodology: The center for care of Covid-19 has been installed in the Internal Medicine department of the Thies Regional Hospital (HRT ), but with dedicated beds for patients non affected by the pandemic . Fifteen doctoral students were assigned, by local medical school , to the Department of Internal Medicine in order to be responsible for the day-to-day management of the OCC. They were supervised by internal medicine specialists . This service was divided into two parts: the OCC that handled Covid -19 cases and the rest of the service , which was to continue to take care of patients with other conditions or who were regularly monitored. Results: From May 1 to October 30, 2020, 237 patients were admitted to the CTE They ranged from 7 to 88 years old with an average age of 53.41 and a sex ratio of 1.60 .The reasons for admission were an oxygen desaturation of less than 90%, the presence of at least one comorbidity (other infections, diabetes, arterial hypertension, obesity, autoimmune diseases, cancers, etc .). Advanced age but also patients who could not be at home were also part of the admission criteria. Three (3) cases of Covid -19 co -infection and pulmonary tuberculosis were identified and three (3) patients had a chronic carriage of the hepatitis B virus. In the context of non -Covid -19 hospitalizations, the following conditions have been found: 8 cases of unbalanced diabetes and as many hepatopathy (6.10%); malaria access in 3.05% (n = 3); pulmonary tuberculosis (3.81%, n = 3); 3 cases (2.29%) of biermeric type anemia and systemic lupus erythematosus. Similarly, 1 case (0.76 %) of cerebral empyema; rheumatoid arthritis (0.76%), one (01) chronic kidney disease, 1 case of heart failure were also recorded Five (5) cases (3.81%) not affected by Covid-19 infection, on admission, were during their hospitalization and therefore transferred to the red zone of the CTE. Conclusion : The establishment of the OCC in the internal medicine service allowed an efficient adaptation in the care of the patients affected by covid disease but also of those who were followed for chronic pathologies or admitted for other medical emergencies This strategy has improved and strengthened the links with other sectors of Senegal 's health pyramid.

Contexte et objectifs. La greffe rénale n?est pas encore pratiquée en RDC, ce qui oblige les patients à se faire opérer à l?étranger. Le suivi post-greffe est fait localement par des médecins congolais, mais n?a pas encore été évalué. Nos objectifs étaient de déterminer la survie des greffons et d?identifier les facteurs associés à la perte de la fonction rénale du greffon chez les patients suivis à Kinshasa.Méthodes. Nous avons colligé les dossiers des patients qui ont bénéficié d?une greffe rénale à l?étranger (86% en Inde) et dont le suivi post-greffe a été effectué dans les hôpitaux de Kinshasa entre 2000 et 2017. Les paramètres d?intérêt étaient : l?âge au moment de la greffe, le type de greffe, la spécialité du médecin qui avait sélectionné le donneur et qui a fait le suivi post-greffe à Kinshasa, la régularité de prise des immunosuppresseurs et l?issue de la greffe (complications, rejet ou non, reprise ou non en dialyse). Les facteurs associés à la perte de la fonction rénale du greffon (défini par le rejet, une néphropathie chronique du greffon ou la reprise en dialyse) étaient recherchés en analyse univariée. La survie des greffons a été décrite par la méthode de Kaplan-Meier.Résultats. Au total, 29 sujets (moyenne d?âge à la greffe : 48±15 ans ; 83% d?hommes) ont été ou sont suivis en post-greffe rénale à Kinshasa. La greffe avec donneur vivant a été majoritaire (97%), avec 7% de donation ABO différent.Le rejet a été diagnostiqué dans 24% des cas. Les autres complications rapportées étaient : la néphropathie chronique du greffon (24%), les complications urologiques ou vasculaires per-opératoires (3%), la nécrose tubulaire aiguë transitoire (7%), les cancers (10%), le diabète cortisonique (7%) et la réactivation d?une infection par le VHC (3%). 0nze patients (38%) ont été repris en dialyse. La prise irrégulière des immunosuppresseurs (0R :3,33 ; IC 95% : 1,71-6,49 ; p=0,002) et le suivi par un médecin non néphrologue (OR: 2,88 ; IC 95% : 1,64-5,03 ; p=0,004) étaient les deux facteurs associés à la perte de la fonction rénale du greffon. La survie moyenne des greffons était de 61 mois dans le groupe entier, meilleure lorsque la préparation/le suivi était fait par un néphrologue (83 mois vs 41 mois ; p=0,026).Conclusion. Nonobstant un faible échantillonnage, ces donnent révèlent une moins bonne survie des greffons après la transplantation rénale chez les patients suivis à Kinshasa, notamment lorsque la prise en charge n?est pas faite en milieu spécialisé. Une prise de conscience collective et la nécessité d?établir des normes et de bien préparer les patients s?imposent

ABSTRACT

The HIV/AIDS pandemic put significant strain on healthcare services in the country. Hospitals were no longer coping with the escalating number of AIDS patients. This resulted in the early discharge of patients; with some patients; too ill to be nursed at home; being sent to hospices for continued care. The Batswana had mixed feelings about hospice care; because their beliefs on patient care are based on the ubuntu philosophy; which emphasises the principle of caring for one another. The purpose of this study was to explore and describe the experiences of Batswana families regarding hospice care for patients in the Thlabane township in the province of the North West as well as to make recommendations to policy-makers to ensure that hospices are accepted by community members and utilised effectively. A qualitative; explorative; descriptive research design was applied. Purposive sampling was applied to select study participants with whom in-depth unstructured interviews were conducted. A qualitative data analysis was done by categorising; ordering; and summarising the data; and describing the findings. The findings indicated that families of patients in hospice care experienced such care as foreign to their culture. These families also experienced stigmatisation; firstly owing to the stigma associated with AIDS and secondly because they opted for hospice care. However; they also observed the high quality of care provided by the hospice and understood its benefits for AIDS patients. The study concluded that hospice care relieved families of terminally ill AIDS patients of the burden of care and enabled them to keep on working and earning a living. Recommendations to policy-makers included enhancing hospice care and ensuring the provisioning of culturally safe hospice care

ABSTRACT

Objective. To study the needs of outpatients suffering from schizophrenia and their primary caregivers. Methods. A qualitative descriptive design was selected to study the needs of a non-probability purposive sample of 50 outpatients with schizophrenia and their primary caregivers. Data were collected on their compliance-related needs as well as psychosocial and aftercare treatment needs. Results. Compliance needs: Eighty-three per cent of the participants supported their medication treatment and aftercare, but needed more information on the illness than they had received or had managed to gather. Psychosocial needs: Participants had difficulty in accepting the illness and in understanding its consequences for their everyday functioning. Ninety-seven per cent of patients were aware that their illness had affected the health of their primary caregiver. The majority of participants had lost friendships since the diagnosis of schizophrenia. Eighty-eight of the primary caregivers supported the patient financially, despite the fact that many were over 60 years of age. Aftercare treatment needs: A strong need was expressed for services such as day-care centres, workshops, and psychoeducational and social work services. Conclusions. As South African health policy is moving towards de-institutionalisation and treating mental health as part of primary care; support must be improved for sufferers of schizophrenia and their primary caregivers. Many people are prepared to face up to the challenge of caring for a mentally ill family member, yet are faced with significant needs that should be addressed

Le Centre Hospitalier et Universitaire de Brazzaville (C.H.U-B), est l'une des grandes structures sanitaires en République du Congo. Dans ce centre hospitalier, on constate une présence massive des gardes ­ malades, qui interpelle plus d'un observateur au point de s'interroger sur leur nécessité. Celle-ci serait ­ elle l'expression de la solidarité ou du dysfonctionnement des services sous tutelle du C.H.U-B? Pour vérifier nos hypothèses, nous nous sommes parvenus au résultat selon lequel, cette présence des gardes ­ malades dans ce centre hospitalier résulte de la conjonction de ces deux (02) facteurs, c'est ­ à ­ dire la solidarité mécanique et le dysfonctionnement du C.H.U-B. À cela s'ajoute l'inadéquation entre les nouvelles attentes des malades et l'état actuel des structures de ce centre.


The Brazzaville Hospital and University Center (C.H.U-B) is one of the largest health facilities in the Republic of Congo. In this hospital, there is a massive presence of sick keepers, which challenges more than one observer to question the reason of their existence. Would this be the expression of the solidarity or the dysfunction of the services under the supervision of the C.H.U-B? To verify our hypotheses, we arrived at the result according to which, this presence of the sick keepers in this hospital center results from the conjunction of these two (02) factors, that is to say the mechanical solidarity and the dysfunction of the CHU-B. Added to this is the mismatch between the new expectations of the patients and the current state of the structures of this center.
